Your generous contributions have been and continue to be the foundation for the Association’s support of the UW Orthodontic Department. Credit toward “club” credit is given upon receipt of payments made using this donation form. Donations may be made directly from an alumnus or on the behalf of an alumnus by another person, such as a referring dentist. The donor receives tax credit and the alumnus receives club credit. If you are making a donation on behalf of an alumnus, please indicate the name of the alumni member who should receive credit to their club level.

Dear Alums,
The purpose of the UWOAA is to support the activities of the UW Department of Orthodontics.
Through your generous contributions, the UWOAA has created an “endowment”, and the interest from these monies is used to fund important and recurring activities. For example, Orthodontic graduate student research projects have largely been funded by the UWOAA. Additionally, the UWOAA has sponsored Thesis Day and the Alton Moore Lecture, and has assisted with the expenses of the ABO exam. And the UWOAA has also hosts periodic alumni reunions and receptions at regional and national meetings.
Some of your donations have been directed towards specific initiatives. Examples of these are the Moore-Riedel Chair and the Kokich-Shapiro Endowment, both of which enrich the educational mission of our Department. Another example is the Orthodontic clinic remodel accomplished in 2011, which was completely funded by alumni donations.
It is obvious that your support of the UWOAA allows the Department to sustain its tradition of excellence.
All donations to the UWOAA are impactful and greatly appreciated, and the UWOAA President is always happy to assist with questions about special initiatives or priorities. There also may be times when you wish to donate directly to the UW in support of the various Orthodontic Funds that exist within the University, like the Graduate Student Fund. The Department Chair is happy to assist with questions about donations directed to the UW. The information below helps describe the nuances of donating to either the UWOAA or the UW.
Tax considerations – Donations to both entities should be tax-deductible. Both the UWOAA and the UW provide documentation of your donations.
Mechanism of donations – For both entities, the easiest way to donate is via a credit card at these respective websites.

However, checks can also be written and sent to the UWOAA or to the Department of Orthodontics. In either case, please be sure to specify what fund you wish to support, or if you wish to support the general activities of the UWOAA. Some alums have chosen to donate via charitable trusts or wish to include the Department in their wills. For these situations, please contact Peter Shapiro or Greg Huang. Likewise, if you are considering a large gift to the UWOAA or the UW, please contact Peter or Greg, as establishing an endowed fund may be appropriate and desirable.
UWOAA and UW Credit – Both the UWOAA and the UW track the lifetime donations of its alums. At various credit levels, donors may receive special recognition and/or perks.
UWOAA DONATIONS: When donations are made to the UWOAA, the UWOAA provides the donor with UWOAA credit. However, UW credit will depend on whether the donation is eventually forwarded to the UW. For example, if you make a general donation to the UWOAA and those monies are kept in the UWOAA “Endowment”, no UW credit is given. However, if you donate to the UWOAA with the specific purpose of having that money used for a UW Orthodontics fund (like the Moore-Riedel Chair in Orthodontics), then the UWOAA will eventually transfer these funds to the UW. At the time the funds are transferred to the UW, the UWOAA will inform the UW who the original donation was from, and then the UW will provide credit to that person.
UW DONATIONS: In general, donations made to any UW orthodontic funds or endowments will result in credit from the UW, as well as credit from the UWOAA. The UWOAA credit occurs because the UW shares donor information with the UWOAA every quarter, and the UWOAA then provides these alums with UWOAA credit.
